Aging as a Masterpiece: Helping Families Navigate and Reframe Conversations about Late Life Transitions

Roundtable discussion will engage a panel of experts in the areas of aging individuals to answer questions and provide practical tips about helping our elderly loved ones navigate their later years with grace and confidence, from healthcare and financial decisions, to obtaining needed care and transitioning to different needs-based living environments.  A topic that is often approached with trepidation, this CLE shifts the conversation towards a positive prism for viewing the fourth stage of life, reframing the later stages as a pinnacle period of life, as a masterpiece.
 
This CLE is excellent for elder law attorneys, including estate planning attorneys who prepare powers of attorney, and all of us who will have to navigate these discussions with loved ones in our personal lives.

Vicki Buchda, Vice President of Care Improvement, Arizona Hospital and Healthcare Association
Vicki Buchda, MS, RN, NEA-BC serves as the Vice President, Care Improvement for the Arizona Hospital and Healthcare Association.  She is responsible for contributing to the organization’s mission: “Making Arizona the healthiest state in the Nation” by leading collaboratives to improve patient safety, patient experience, healthcare outcomes, and health equity. She is a highly skilled professional with demonstrated expertise in clinical and system leadership. Vicki is passionate about harm prevention, including harm from opioids, maternal health, advance care planning and health equity. Vicki has served in nursing and leadership roles within health systems including Mayo Clinic, Hawaii Pacific Health and Banner Health.  
Melissa Cardine, Patient Advocate, Bridge Health Advocates
Melissa Cardine MSN, RN, BCPA has been a nurse for 25 years and is the founder of The Bridge RN Patient Advocates. She enjoys listening to patients, families, and caregivers along with collaborating with healthcare professionals. She has found tremendous success as a private patient advocate and helped numerous clients have a better experience along their healthcare journey. Melissa co-founded The Bridge Health Advocates providing health advocacy and business training for those interested in learning more about professional advocacy work.  Melissa was selected to be a Board Member of the Patient Advocate Certification Board in November 2019 and served as President from 2020-2021.  Her passion for advocacy is evident in her daily interactions with all those she meets.
Alyssa Crockett
Alyssa Crockett serves as Senior Leadership and Legacy Officer for Valleywise Health Foundation, overseeing community grants, strategic partnerships and planned giving. An innovative, growth-oriented leader with 20+ years experience in health care, education and campaigns, Alyssa held leadership roles at Boys & Girls Clubs of the Valley, Ryan House, Hospice of the Valley and Xavier College Prep. Originally from Washington, D.C. she is a proud Arizona Wildcat and mother of three.
Andrea Garcia
Having grown up in a tight-knit family with many older adults, Andrea Garcia developed a strong passion for the aging population. As a real estate agent, Andrea assists with all matters involving adult children helping their aging loved ones transition from their home to a safer environment - an independent living community, assisted living community, memory care community, or a continuing care retirement community. 

As a dedicated resource for families and seniors during the end-of-life cycle and beyond, Andrea specializes in seeing the transition through, from beginning to end, by working closely with family members and associated professionals with integrity and efficiency. 

Her experience as the Client Services Director for a local home care company has enabled her to be a key resource for both families and professional partners. It was her responsibility to interact with community affiliates and, most importantly, visit with family members to assess and develop a care plan for their aging loved one, so they may continue to remain at home. Andrea has served the aging population and their adult children, specifically as it relates to downsizing or rightsizing for 5+ years, but has helped countless families in this realm for 10+ years.  
Katie Jensen, Director of Development, Valleywise Health Foundation
Katie Jensen serves as Director of Development for Valleywise Health Foundation, which supports the public teaching health system for Maricopa County. Katie began her career in non-profit in the Midwest before coming to Arizona to join Sojourner Center, which provides shelter and support services to women and children escaping domestic violence. Katie has a strong commitment and passion for serving and giving back to the community. She holds a B.A. from Indiana University Bloomington.
Nora L Jones, Tiffany & Bosco PA
Nora Jones is a skilled trial attorney specializing in probate and trust litigation, elder law, guardianships and conservatorships, and estate and trust administration. A recognized leader in the elder law community, Nora blends compassion and strength to resolve matters with her clients’ best interests at heart.  Nora is recognized by the Arizona Board of Legal Specialization as a Certified Specialist in estates and trusts.  In addition, Nora has an AV Preeminent Rating, the highest possible rating in both legal ability and ethical standards, by Martindale-Hubbell and has a Perfect 10 rating with AVVO.  Nora has been recognized year after year as a Rising Star by Super Lawyers and has repeatedly been honored for her vision, influence, and leadership in Elder Law by Arizona Business Leaders. Additionally, Nora and her team have been recognized as a top law firm for Trust and Estate Litigation by U.S. News & World Report. Nora is also an acting Judge Pro Tempore with the Superior Court of Maricopa County for the Probate Court division.
Sara Wilson, CEO, Home Health Assist
Sara Wilson has worked in home care for 20 years and has served the last 8 years as CEO of Home Assist Health (HAH), a Phoenix-based non-profit that offers personalized home- and community-based services to seniors and individuals with disabilities. HAH offers a variety of services including personal care assistance, companionship, housekeeping, respite care, habilitation, enhanced care transitions and chronic disease management. 

A passionate advocate for individuals with complex health needs, innovation has come naturally for Sara.  She has spent two decades responding to the changing needs of the folks she serves with creative solutions.  Her continued efforts have helped Home Assist Health become one of the most innovative agencies in the Southwest. Through her leadership the organization has become a bridge of compassionate equality for diverse populations through valued partners including Valleywise Health SystemHealth CurrentMercy Care Long Term CareUnited Health Care, Banner University Health Plans, and The Department of Economic Security – Division of Developmental Disabilities.

Sara and her team are the frequent recipients of numerous industry and business accolades. Her leadership is also reflected in the national and local boards she sits on including the Partnership for Medicaid Home Based Care, Arizona Home Care Association of America, and the Mayo Community Advisory Board. A graduate of Arizona State University Sara earned her bachelor’s degree in Interpersonal and Organizational Communication and her master’s in Business.
